*If you are receiving your pump through Commerical Insurance, you may be responsible for deductibles and co-pays on certain plans.
HOW TO REQUEST A BREAST PUMP

Have your health care provider send a prescription via fax to (612) 353-6437 or via email to abby@everyday-miracles.org
Once your request had been approved, the breast pump will be shipped directly to your home address

MEDELA:
The Pump In Style® with MaxFlow™ personal double breast pump starter set is 100% covered ($0 out-of-pocket)
The Medela Freestyle Hands Free is covered with a $110 out-of-pocket copay that is paid through our online store
*If you prefer a different model from Medela, you may request this in the “Comments” section of your request form; however, please note that for any other model there will be an additional cost, which varies with pump model.
SPECTRA:
The Spectra S2 model is 100% covered ($0 out-of-pocket)
The Spectra S9 Portable model 100% covered ($0 out-of-pocket)
The Spectra S1 model is covered with a $55 out-of pocket copay that is paid through our online store
The Spectra SG , Spectra SG Portable, and Spectra SG Bundle is covered with a $100-120 out-of pocket copay that is paid through our online store
